Note: Per-Patient Alerts need to be enabled for your clinic by the CoachCare Tech Team. Please reach out to your Provider or Support to use this feature.
The current patient alerts are now referenced as "Clinic-Wide Alerts." For alerts not fitting a "one-size fits all" approach, you can now create an Individual Per-Patient Alert from the Patient Chart.
Creating a Clinic-Wide Patient Alert
When this type of alert is set up, it will apply to ALL Patients in the clinic.
For example, you want to set up a "Heart Rate" Threshold Alert when ANY patient exceeds 100bpm
Click the following article to create one: Clinic-Wide Patient Alerts.
Note: If a clinic-wide alert is not created for a per-patient alert, a prompt will appear to automatically generate the clinic-wide alert in a disabled state.
Creating an Individual Per-Patient Alert
When this type of alert is set up, it will apply to an Individual Patient in the clinic.
For example, you want to set up a "Heart Rate" Threshold Alert specifically for ONE patient when they exceeds 95bpm
Note: The Individual Per-Patient alert of 95bpm Threshold Alert will supersede the Clinic-Wide 100bpm Threshold Alert.

Navigate to the Patient Chart in Monitoring Mode or Legacy Mode
In monitoring mode, ensure the toggle is selected above -
Go to the Alerts Card and Click on "Add New" in the upper-right corner of the card
Note: To confirm Per-Patient Alerts are enabled for your clinic you will see (Patient-specific alerts enabled) in the Alert Card Title
In Legacy Mode, you will see (Per-Patient) next to 'Recent Alerts'

Select the "Alert Type" to Create
There are multiple alert types for different use cases. Here is a quick summary of each:- Threshold Alert - This alert can be configured to be activated based on a data type, an upper or lower limit, and a refresh period.
- Missing Data Alert - This alert can be configured to generate a notification when a patient has not recorded data for a metric in a specific number of consecutive days.
- New Measurement Data Alert - This alert can be set up to notify when a new measurement data is created. Refresh period is set up to one hour.
- Reminder Alert - Set this alert with a custom typed note to notify you at a specific time.
- Delta Change Alert - This alert notifies when a delta change is registered outside a threshold from the current time value to a future time’s measured value.
- New Form Submission Alert - This alert can be set up to notify when a form is submitted by a patient.
In our example we will use the Threshold Alert.
4. A "Modal" will pop-up to enter the Alert Type Details
Each Alert Type will have its own requirements (future articles will be referenced and linked here).
In our example, we've set the Threshold Alert to notify when this patient's Heart Rate exceeds 95bpm.
Review the confirmation at the bottom of the modal and click "Save" when complete
Note: All Active Alerts related to this patient and alert type will be referenced (Organized by Clinic-Wide and Per-Patient). You will see the Clinic-Wide Alert we mentioned before when ANY Patient exceeds 100bpm.
Reviewing and Editing an Individual Per-Patient Alert
1. Click on "Manage" in the upper-right corner of the Alert card
In Legacy Mode, click "Per-Patient Alert Settings"
2. A separate page will load with an "Alerts" and "Alerts Settings" tab.
The "Alerts" tab will show the current Active Alerts with notifications.
Here we can see an example of the Per-Patient alert we created triggered when exceeding 97bpm when the Clinic-Wide alert was set for 100bpm being exceeded.
3. Click the "Alerts Settings" tab to view and edit Per-Patient Alerts
Note: You will see the Clinic-Wide Alerts in a lighter gray color. These are editable form the main Nav Menu > Alerts > Setting page. Per article Clinic-Wide Patient Alerts.
To edit the Per-Patient alerts, you will scroll down until you see text in black. Cllick the pencil icon.
Note: As mentioned in the first section for "Clinic-Wide" Alerts, you will see the following prompt to automatically generate a clinic-wide alert in a disabled state.
All names and identifying information included in this guide are fictional.
