Setting Up New Measurement Data Alerts at Patient Level in CoachCare

This guide provides a straightforward method for setting up personalized measurement alerts in CoachCare, ensuring clinicians can monitor patient metrics effectively. By following these steps, users can quickly create alerts tailored to individual patients, improving care management. The process also includes automatic clinic-wide alert creation for unconfigured metrics, enhancing overall functionality. This resource is essential for optimizing patient monitoring and ensuring timely responses to critical health data.
 

1. Navigate to Coachcare patient account.

2. Click "Add New Alert" button

3. Click "Alert Type" dropdown to choose 'New Measurement Data Alert'

4. Choose "New Measurement Data Alert" from the dropdown

5. Choose the required Metrics to alert. Example: "Heart Rate"

6. If a particular 'New Measurement Data Alert' is not configured for the clinic, a message will appear stating: 'A clinic-wide alert will be created automatically and set to disabled, as one does not currently exist.'

7. Click the 'Save' button

8. To confirm again, a pop-up window will display the message: 'This alert will only apply to this patient. A clinic-wide alert will be created and disabled to enable this feature. Click "Yes"

9. The 'New Measurement Data Alert' has been created for the patient. To confirm, click on 'Per-Patient Alert Settings.'

10. Check "New Measurement Data Alert - Heart rate" is enabled

11. Click "close" button to exit the window

Setting Up New Measurement Alerts at Patient Level in CoachCare - Monitoring Mode

1. Navigate to CoachCare patient account - Monitoring Mode view

2. Go to the 'Alerts Card' and click the 'Manage' button

3. You will be able to see an 'Alerts' and 'Alert Setting' tab

4. Click the 'Add New' button

5. Choose "New Measurement Data Alert" from the drop-down

6. Click the Metric to show the drop-down options

7. Choose the required Metric to alert. Example: "Glucose"

8. If a particular 'New Measurement Data Alert' is not configured for the clinic, a message will appear stating: 'A clinic-wide alert will be created automatically and set to disabled, as one does not currently exist.'

9. Click the "Save" button

10. To confirm again, a pop-up window will display the message: 'This alert will only apply to this patient. A clinic-wide alert will be created and disabled to enable this feature.'

11. Click 'Yes.' The 'New Measurement Data Alert' has been created for the patient.

12. To confirm, click on Per-Patient 'Alert Settings' Tab to view the created alert

13. Click the 'Close' button to exit the alert window.
